Long Term Conditions

If you have a long term medical condition such as Asthma, Diabetes, Chronic Kidney Disease, Rheumatoid Arthritis or one of several other conditions, you will usually need to have a regular check-up at least once each year. Most of our annual check-ups are done by our Nursing Team and we will contact you when you need to be seen and advise you who to make an appointment with.

If your symptoms change, you feel your condition is getting worse or your medication is no longer effective do please make an appointment to see your GP at any time and don’t wait until your annual check-up.
